| The Care Giving Team |

| The Care Giving Team consists of a group of professionals who care for patients' needs on all levels: medical, physical, emotional, and spiritual. Their purpose is to provide support and keep the patient as pain-free as possible, with loved ones through the end-of-life process. The Hospice Care team creates a care plan to meet the individual needs of each patient for pain management and symptom control. |

Registered Nurse

 |
A registered nurse is assigned to educate patient and family, offer family support services and coordinate all services reasonable and necessary to the patient’s illness. The nurse will work closely with the patient’s physician to ensure he/she receives the appropriate care. |

The Attending Physician – participates in the Plan of Care and remains the patient’s primary physician. |

Hospice Medical Director

The Hospice Medical Director – oversees the Plan of Care and advises the hospice team. |
|
Hospice Aide
|
 |
A hospice aide/homemaker provides personal care and assistance with activities of daily living, feeding and bathing, and hands-on care |
|
Bereavement Counselor
|
 |
A grief and bereavement counselor provides help and comfort during hospice care and for thirteen (13) months after the patient’s death. |
|
Social Worker
|
 |
A social worker helps deal with psychosocial needs of a patient/family facing terminal illness. Services are coordinate with patient’s physician and community resources. A social worker is available to assist with legal, financial and social resources. |

Spiritual Care Counselor
|
 |
Hospice Care understands how difficult this time can be for those who are faced with a loss of a loved one. Our Spiritual Counselors offer support services ranging from crisis help to private and group grief support. |
|
Volunteers
|
 |
Hospice volunteers help us provide the extra love and care our clients and their families need. Volunteers offer support, companionship and practical, caring help. Whether it’s running errands, staying with the patient so family members can take a break, reading to patients, or holding patients' hands, we will make available as needed the services of trained volunteers.
